Our Mission

LucidAct Health’s mission is to make smart care at home accessible to every person in the world.

Our History

As primary caregivers tasked with coordinating medical care to our aging family members, we have firsthand knowledge of the difficulty in bridging hospital care and home care after discharge. We wanted physicians and care teams to be able to understand each patient’s situation more deeply and intimately. And we wanted patients to be able to participate in remote monitoring with minimal effort. As Silicon Valley veterans with experience at companies like IBM and hospital systems like Stanford Healthcare and Sutter Health, we knew what it would take to build a platform like this. And so, LucidAct Health was born.

Today, LucidAct is the leader in Connected Healthcare, empowering healthcare teams to provide proactive, truly personalized care and avoid unnecessary hospitalization after discharge. The connected care experience leverages LucidAct's AI technology by bringing together and analyzing data from multiple EMR systems, in-home monitoring devices, and insurance claims history to deliver better outcomes, higher patient satisfaction, and generate additional revenue for providers.
